Four talented actors perform completely unrehearsed skits and games in front of a studio audience. Host Drew Carey sets the scene, with contributions from the audience, but the actors rely completely on their quick wit and improvisational skills. It's genuinely improvised, so anything can happen - and often does.

The show is slightly based on the original concept of Extreme Makeover, where individuals receive life-changing makeovers. The exception is that this show focuses primarily on participants losing massive amounts of weight over one year and receiving plastic surgery to remove the excess skin from their transformations.

Holly and Jessica are back together and look for a gimmick to capture the anime crowd. And Yaya returns from Denmark for a photo shoot for her calendar, but collapses halfway through when her corset is too tight.

This episode follows the story of Salvador Magluta and Augusto Falcon, a pair of powerboat racers in Miami who allegedly moved more illegal narcotics than just about anyone in the history of the U.S. War on Drugs. Known as Willy and Sal, they were tried for importing more than 2.1 billion tons of cocaine over more than a decade of trafficking. They were acquitted, but then faced charges of jury tampering. The law finally caught up with Willy and Sal. Falcon plead guilty to obstructing justice and received a 20 year sentence while Sal faced a jury and is now serving 195 years in prison.
