Before the surgery his wife was only somewhat supportive, but the results may be too much for her to handle. Izzie tries to go back to the hospital for the first time since Denny died. One child's constant visits are noted by a nurse. Two foster patients are met with suspicions of child abuse. George and Callie's relationship moves forward.

In Dallas, a young woman is found stabbed, beaten, and burned on a bed. The victim's 12-year-old niece and her mother are missing. Lead Detective Dale Lundberg soon discovers that this murder is extremely personal. In Memphis, Sgt. Mitch Oliver is investigating the homicide of a pool hustler found dead in a seedy motel room. The victim's car is nowhere to be found. Oliver hopes if he finds the car, he'll find his suspect.

Tony Hill assists the police in their investigation of a bizarre series of murders. In each case the victims - a property developer, a bishop, a judge - were brutally killed and the killers then immediately committed suicide. Tony thinks there is a connection to the Freemasons, but then realizes that they are dealing with a religious sect that glorifies suicide. He knows from the profile that the head of the sect will be a white male and from the religious symbolism that he will have six disciples, but who are they and how will they find this needle in a haystack?
