Domenico Clericuzio, the family Patriarch exacts revenge on the Santadio family by ordering the execution of the entire Santadio family, including his new son-in-law on Rose Marie Clericuzio's wedding night. After her brief wedding night, Rose Marie is filled with grief and learns she is pregnant. She gives birth to her son Dante and as he grows, he is instilled with an evident hatred for his grandfather. In turn, the family becomes increasingly aware of his taste for blood.

Meanwhile Walker and Trivette learn from Tony's sister, Mollie, that he had falling out with Tillman, and with Gordon's help they suspect Tillman is behind the murder - because if the prosecuting attorney is having an affair with the defense attorney and the defense attorney ends up dead then Lane Tillman would go free because there would be no second trial. Cox finds something interesting about the Felton case; she digs in deeper and the results change the case to a murder. Bayliss turns up the next day and disappears again to his Uncle George's place. Pembleton and Howard try to work with Falsone to find Beau's killer. Gee finds out that Beau had been working undercover for the past six months on the Cantwell auto-theft ring. In the Brighton Beach area of New York City, a man named Yaponchik kills a Russian Elder by shooting him in the face with a 12 gauge pistol. It becomes the third such killing in which the perpetrator mutilated the body to prevent positive identification. Frank assists N.Y.P.D. Lieutenant McCormick, Yura Surova, from the Moscow Police Department, and undercover officer Andrei Medikov with the investigation. While inspecting the Elder's corpse, Frank notices an odd mark in the shape of a inverted "V." But he is unable, at first, to determine its meaning.

Ed tracks down Khadijah at the slot machines, and admits that he has made many mistakes in their relationship. However, it is important that Khadijah remain part of his life, as she is the only thing that truly brings him pride. He apologizes for not having a greater role in her development. Khadijah wins the jackpot with Ed's coin. Max threatens to harm Overton if he tells anyone about her affair with Kyle. Synclaire realizes he is keeping a secret. When Regine can't get Overton to crack, Synclaire confronts him. He tells her about Max and Kyle, but she isn't surprised; she has the room below Kyle's!
