Medical Examiner Dr. Ian Michaels (Tom Irwin) conducts the autopsy with the help of his assistant, Ollie Gibb (Don MacKay). He finds an unusual bullet-like projectile lodged in the victim's brain.

In addition to the name change to "Oswald State Correctional Facility," more changes abound. Newbies Coyle, Kahn, and Ricardo arrive and partner up with Wangler, Said, and Hernandez respectively. Adebisi's "recovery" prompts a return. Clayton Hughes, son of a past Oz guard who came through the ranks with Glynn, asks Glynn to hire him. Meanwhile new Officer Howell, gets friendly with McManus. A managed health care company takes over Oz's infirmary, to Dr. Nathan's disgust and Alvarez's detriment. Cyril O'Reily still has nightmares about being raped by Schillinger and Ryan schemes on revenge. Said develops an unlikely alliance as he starts a hunger fast in response to McManus rejecting his request for food in his cell during his holy season. Keller confesses his part in setting up Beecher and is immediately taken into protective custody. Beecher gets some oh-so-sweet revenge.

Sean proposed to Helen but she left for work, leaving him to ponder. Shell and Denny kicked Zandra and made her tell them who her supplier was - then they made her lure Lorna into the toilets. Lorna declined to get Zandra more drugs until Shell emergedfrom the cubicle and threatened her. Lorna then smuggled more drugs in and was nearly caught by Fenner. Spencer visited Monica but looked ill. Nikki was moved into a new, better room as a privilage. Sean did a presentation at the prison where he was introduced as Helen's partner; much to Nikki's disgust. After seeing inmates entering and leaving Shell's cell, Dominic investigated and found drugs in the Budgie's cage. Shell then shouted that it was Lorna who supplied them - only Fenner beleived her though. Shell had her priveliges removed after the drugs incident so took out her anger on the budgie - poor little thing!

Red's mother Bernice has once again managed to make everyone miserable during one of her visits. While driving his grandmother home, Eric lashes out and tells her that it wouldn't kill her to be nice once in a while. Bernice promptly drops dead, leaving Eric to think he killed her. The entire Forman family (including Laurie and Red's brother Marty) comes together for her funeral while Eric gets drunk at a bar with the guys.
