Cassandra surprises John when she suddenly shows up at the office. She wants to continue the relationship they started in LA, but John doesn't want to go on seeing her, because they're 3.000 miles away and he's not completely over Melanie yet. Ally takes the case of a man who's being sued because he didn't reveal to his internet date that he was a dwarf. Larry is representing the woman suing, and the case ends up reflecting on their own relationship as Ally questions Larry if they would be together if she was 3 feet tall. Ling and Jackson take a case of a woman who's suing her old boyfriend for interrupting her wedding ceremony. Jackson reveals his true feelings towards Ling, who is confused and torn between him and Richard. Caught in the middle of this is Renee, who feels used by Jackson.

Sean, J.D. and Kekoa travel to the Big Island to help with the extra work caused by the Ironman Triathlon. They soon have a run-in with former lifeguard Jessie Owens, who is also competing in the Triathlon and has new infant son with her. This causes some awkwardness in J.D., and slight jealousy in Kekoa who suspects that J.D. never got over her.

Matt also notices the strange demeanor of his psychologist, which leads to a surprising twist. Meanwhile, after Ruthie gets into trouble at school for missing homeroom classes daily, she tricks Robbie into coming to talk to her principal. Simon decides he no longer wants to be together with Sasha since they have nothing in common and that she is not that intelligent, but the plan backfires on Simon. Also, Mike wants to try to be in a relationship with Lucy again, but unfortunately she turns him down since they are both on the rebound from Jeremy and Elaine. Lastly, Eric must convince a terminally ill patient to embrace life no matter what the doctors tell him.

Until she gets ticked off after Greg tells her she's not fun like Christine, that is. So Kim goes to Jimmy and asks him how to be fun. Jimmy takes her back to the bar and helps her loosen up around other people -- particularly Greg.
